The form asks for your name, enquiry type, and a short message. Google Forms collects your signed-in email address so ILAI can reply. Include an organisation or approximate event date only when relevant.
Your response is received by ILAI through Google Forms for reviewing and replying to your enquiry; Google hosts the form. Please do not include patient information, clinical records, confidential institutional documents, passwords, or other sensitive data. This is not a clinical consultation or emergency service.
